Purchase Request Form

Use this form to request the purchase of a book, serial, DVD, CD, video or electronic resource you would like to see added to the ISU Library collection. The relevant library department liaison and the requester will be e-mailed a copy of this request. After verification of the bibliographic information, the liaison will forward the purchase request to the Acquisitions Department. The department representative will be consulted.
